Initial clinic appointments for children are made through Cheryl, the pediatric epilepsy nurse coordinator, at (504) 896-9859. Please use the patient referral form below for the Children’s Hospital Epilepsy Center and fax it to Cheryl at (504) 896-9547.
Neurodiagnostic testing (EEG, Epilepsy Monitoring) is managed through the Neurodiagnostics Department. The Director is Cherie Young and the contact phone number is (504) 896-9596.

Location
The Pediatric Neurology Clinic is located on the 3rd floor of the first tower building (Ambulatory Care Center) on Henry Clay Avenue and Tchoupitoulas Street. Patients should use the green elevators to gain entrance to the patient clinics.
