PLANNING THE MUSCLE BIOPSY

Designated person should call the Muscle Lab at LSUHSC (504)599-0281 in advance of scheduling the biopsy to make sure that someone will be there on that day to receive the muscle biopsy. Lab hours are 8a.m. to 4p.m., Monday through Friday.(Closed weekends and holidays.) Call the LSUHSC Pathology Department, Muscle Lab at (504)599-0281 or 568-6042 and ask to speak to Cheryl Vega or Teresa Flores. Please call again when you actually send the muscle biopsy to make sure someone in the lab will be waiting to receive the specimen.

PREPARATION OF THE MUSCLE BIOPSY SPECIMEN

The surgeon should remove a piece of muscle approximately 1.0x0.5x0.5cm. Ideally it should be cut so that the longitudinal orientation of the muscle fibers is easy to see. Place the muscle tissue in a clean, dry closed container, such as a screw-cap vial. DO NOT put the muscle in saline or wrap it in wet gauze, because saline washes out the glycogen. It is alright, however, to enclose moist gauze in the container, but not touching the muscle, to keep it from drying out. DO NOT tie the muscle with sutures, clamp it or stretch it on wood. The muscle tissue should then be sent to the hospital Pathology Lab, to be properly recorded and assigned an accession number. The specimen container is then packed in another container with crushed ice or ice cubes in it. This is to keep the specimen cool (refrigerator temperature) without freezing it. If you need to use ice packs instead of wet ice, put some paper towels between the specimen container and the ice packs so the muscle will not accidently freeze in transit. Enclose a completed, LSUHSC Muscle Biopsy Requisition Form with the specimen and send it to the LSUHSC Muscle Lab, Rm. 5D22, 1901 Perdido St. New Orleans, LA 70112 as soon as possible. Ideally a muscle biopsy specimen should reach the LSUHSC Muscle Lab within two hours of the surgery. Most hospitals pay a taxi or courier service to deliver the specimen. The delivery person must bring the package containing the muscle biopsy to Rm# 5D22, as the University Police will not accept packages. If it is necessary to send a specimen by overnight mail, package it as described above, using ice packs instead of wet ice and place container in a styrofoam box. DO NOT mail an overnight package on Friday.

THE FOLLOWING INFORMATION MUST BE ON THE REQUISITION FORM

Name & address of the hospital or clinic from which the specimen is sent
Name, date of birth and sex of the patient
Hospital identification number (Medical Records #)
Pathology specimen number
Which muscle was biopsied (or the location of the muscle biopsied)
Date of collection of specimen
Name and phone number of requesting physician
Clinical diagnosis or symptoms (History)
Name and address of physician to whom the report and slides should be sent if different from the Pathologist of the hospital that submitted the muscle biopsy.

ENCLOSE THE COMPLETED REQUISITION FORM (in a plastic bag) IN THE PACKAGE AND SUBMIT TO:

Dr. Hernan Correa,a Board Certified Pathologist, will read the prepared battery of muscle enzyme stains and a report, photographs and a set of slides will be mailed to the Pathology Department of the hospital that submitted the muscle biopsy or the requesting physician within two weeks of receiving the muscle biopsy.

LSUHSC Muscle Laboratory will bill the Hospital that submitted the muscle biopsy a charge of $750.00. If Dystrophin Immunohistochemistry is required by Dr. H. Correa for proper diagnosis, there will be an additional charge of $50.00 per antibody.

Lab hours are 7:30am-4:00pm Monday through Friday. (Closed weekends and holidays). Call the LSUHSC Pathology Dept., Neuromuscular Lab at (504)599-0281 or 568-6042 and ask to speak to Cheryl Vega or Teresa Flores.
Please call again when you actually send the nerve biopsy specimen to make sure someone in the lab will be waiting to receive the specimen.

PREPARATION OF THE NERVE BIOPSY SPECIMEN

Whole nerve biopsy rather than fascicular biopsy is preferred. Usually sural nerve is taken, but the superficial radial nerve at the wrist is also accessible. To minimize patient discomfort, the nerve should first be divided proximally. It then may be carefully dissected free and the distal end cut without futher pain. After removal, the nerve should be laid straight and stretched on a piece of tongue blade by carefully pulling on the ends of the nerve with forceps and allowing it to adhere for a few seconds. Pinning is not necessary. The piece of wood should be placed so that the nerve is on the down side and submerged in the fixative. Do not delay fixation or allow the nerve to dry out. Either paraformaldehyde EM fixative or 10% neutral buffered formalin may be used to fix the nerve. The nerve specimen should then be sent to the hospital Pathology Lab, to be properly recorded and assigned an accession number. The specimen may be sent to LSUHSC Lab the same day or allowed to fix over-night, removed from the wood, and then sent. The latter is preferable because the nerve is more likely to remain submerged in the fixative while in transit if it is not on the wood. Most hospitals pay a taxi driver to deliver the specimen

Name, age, birthday and sex of the patient.
Hospital or Clinic identification number (Medical Records #).
Pathology specimen number.
Which nerve was biopsied (or location of the nerve biopsied).
Date of collection of the specimen.
Name and phone number of the referring physician.
Whom the report and slides should be sent to.

Our Neuromuscular Lab generally mails the report and slides to the Pathology of the hospital that submitted the nerve biopsy.

Enclose the Requisition Form and specimen in separate plastic bags and submit to:
