Application Information for Pain Fellowship

The Louisiana State University Physical Medicine and Rehabilitation Staff invite you to join our program. We are eager to share our program, medical center, and unique city with you.

The following are guidelines to help you in completing your application to our program:

An applicant must have a valid license to become eligible to practice medicine in the State of Louisiana. Please refer to www.lsbme.org for more details

Requirements:

• Curriculum Vitae w/ recent photograph
• Copy of Dean's Letter
• Copy of Official Medical School Transcript
• Board Scores (USMLE parts I, II, III)
• Personal Statement
• Three letters of recommendation (minimum)
• Verification Letter
• Internship & Graduate Certificate

Personal Interviews are granted by a selection committee and required before acceptance. Please allow a full day for your interview.
