Step 3 Checklist

• Decide a month to take Step 3 you will begin the application process 4-8 weeks prior to that month. Once you finish the process you will have 3 months to take the exam.
• Check to make sure you are eligible for Louisiana License State Requirements - Scroll down and click on Louisiana for specific requirements
• Apply for Step 3 via the website at FSMB - Click on Exam Services the Step 3 Homepage Be prepared for the cost - $655.
• Get signature page in mail - have a passport picture available and sign with the notary. Most of the hospitals we rotate with have a Notary on site, and it's usually free for physicians - check with the operator for their location
• Complete the FSVS application at FSMB - click on Step 3 request. Part of this is to release old Step 1 and 2 scores - which costs $50. Since FSVS is a new requirement, I am unclear if the actual FSVS application needs to be completed (which would be an additional $370)
• Send check to LSBM - $282. They'll automatically send you a letter and return envelope in the mail.
• Get the orange scheduling request in the mail - you now have 90 days to schedule and take the test.
  Total cost: $987 - 1357

State Licensure-Louisiana State Board of Medical Examiners
Your state license number will come in the mail automatically after the state receives your passing scores - you will need two letter of references that you can get from any of your attendings.
CONGRATULATIONS! You can now moonlight!
Remember: The license is good until your birthday - do not begin this application process two month prior to your birthday, because you will have to turn around and pay it again ($150) for the next year.
If you haven't taken Step 3 by June 20th (ie, received your passing results in the mail): You will need a letter from Dr. Baier in order to renew your internship card. The renewal fee is $50. Your intern permit expires June 30th - don't wait until June 30th to renew. You will need your new Permit in order to begin work on July 1.

DEA Application-Louisiana Board of Pharmacy

• Apply for state CDS license first . Cost: $20 and needs to be mailed in.
• Once you have been approved for the state license, you can apply for a Federal DEA number . Select Form 224. Cost: $551 - will only take credit card if you do it online, otherwise mail it in with a check
