Most forms on the LSU New Orleans School of Medicine website should be filled out by the supervising senior physician on the research project. Below are links to the most requested forms that students and residents can fill out on their own. For help filling out any of these forms or help finding forms not listed below, please contact us @ Baton Rouge Campus Research Help Request Form.
**Medical Students and residents must collaborate with a supervising physician on all research projects.**

- HIPAA authorization
- Feasibility Forms (aka Endorsement Letter). This is for any OLOL Research or QI project. Fill out prior to IRB forms.
